A woman suddenly wakes up in the arms of a man who is carrying her away from what the site of a plane crash. Neither of them remembers what happened, nor do they recall knowing each other. Both have the same tattoo of the number 23 on their bodies and wear a bracelet that cannot be removed, with a countdown in progress..The man seems to be mute.They are on a deserted island. Initially, the man takes the initiative, searching for water and food, building a shelter, making rudimentary weapons, and lighting a fire, taking care of the woman. He is very protective and reassuring with her. It becomes a race for outdoor survival, trying to figure out what really happened. They return to the plane and notice a charred body in the pilot's seat. The plane is very small and had only two seats. The island is strangely deserted with no lifeforms but the insects. Together, they try to gather wood to build a raft. Meanwhile, the woman starts feeling sick, experiencing nausea and vomiting. The man goes hunting and gets injured. She will have to take care of both of them as the man worsens and develops a strange fever. She builds a large wood pyre on the beach, to light if they see a ship. With improvised harpoons, she catches fish, finds fruit, and repairs their shelter. Then, something crawls out of the water during the night. The two face undead beings, initially mistaking them for castaways, but they soon become a new nightmare. Between boiling water, staying silent, searching for food, and evading the monsters, their fate seems sealed. The man confronts the undead and manages to eliminate some, but a bad fall from a cliff seems to be his end. The woman makes one last desperate attempt during the night, lighting the pyre on the beach, trying to evade the monsters concentrated in that area. She decides to plunge into the sea and swim away from that place of death. She is rescued by a small boat. The men seem to know her and tell her they saw the fire. They explain to her that she is pregnant and the fetus has a blood strain immune to the epidemic that turned dead into zombies, but needs the second part of the vaccine. What is left of human race, lives on boats and she had only a few days to get the complete cure. The countdown is now finished. Her rescuers tell her the other boat sent just her and the child's father with a small plane. She realizes everything that happened on the island was her hallucination, and Jack was never really with her, but it was as if he had helped her survive to save her and her child. They were the vaccine test couple numeber 23. The ship sails towards the horizon with hope on board.
